The part of the spear you're referring to is just above the o-ring/seal, right on the inside of the keg. This portion of the spear typically produces build-up over a period of time as a result of the beer spilling-out of the coupler during the kegging process. A keg washer won't be able to properly remove this grime so you'll have to completely remove the spears occasionally in order to reach this area and ensure it gets clean.
